Forum NameChange Issue

I'm trying to create a ticket to change my name on Forum and when I press Submit Ticket or whatever, nothing happens, I tried like a few weeks ago too, same thing.

If an admin does read this, I just want my name changed from Luke McCarthy to Toner

Have you fulfilled both the requirements?
- The message must be at least 50 characters.
- The category field is required.
