Apologies.. I have been busy,

I've verified your donation (http://puu.sh/oihav/fee8131cbb.png) from the old boards and have as such changed your forum name to your requested forum name of cataplasia.

For any other people reading this wishing to achieve the same result, please make a support ticket on the UCP as it is much quicker, easier to manage and we can also then inform you (also with an automated e-mail) when your request is complete, to avoid being locked out of the forums.
